Ángela Vidal, the television reporter who survived the outbreak inside a Barcelona apartment building, wakes aboard an old oil tanker anchored far out at sea. The ship has been converted into a quarantine and research facility, where doctors and armed soldiers are meant to study people who carry the infection without yet showing its symptoms. Ángela is held there in isolation under the supervision of Dr. Ricarte, while the crew believes the water around them keeps the danger contained. What they do not know is what she carries inside her, and that the outbreak is far from over.
Jaume Balagueró directed and co-wrote the film with Manu Díez, returning to the series he had made with Paco Plaza. Released in 2014, it is the fourth entry in the franchise and follows Sleep Tight in his filmography, coming before Muse, The Vault and Venus. Manuela Velasco again plays Ángela, the role she originated in the first film, joined by Paco Manzanedo, Héctor Colomé, Ismael Fritschi, Críspulo Cabezas and Mariano Venancio among a credited cast of 23.
Made for about 3 million USD and taking roughly 5 million USD worldwide, the film performed modestly. On IMDb and TMDB it sits at 5.6/10 from around 1,300 votes, close to the critics' rating of 5.2/10 from 21 reviews, indicating a mixed reception from both audiences and reviewers.
