David, a respected neurosurgeon, is dealing with a serious medical condition and sets out to find a treatment. His search leads him to uncover a dark secret connected to the past, drawing him into a story that reaches back to the years of the Spanish Civil War and to a group of children born unable to feel physical pain. The film moves between David's present-day investigation and the earlier period, weaving the two strands together as the connection between them becomes clear.
Painless is a Spanish, Portuguese and French co-production directed by Juan Carlos Medina, who wrote the screenplay with Luiso Berdejo. It was Medina's first feature, followed by The Limehouse Golem in 2016 and The Chase in 2024. The cast includes Àlex Brendemühl as David, Tómas Lemarquis as Berkano, Derek de Lint as Professor Holzmann and Ramon Fontserè as Dr. Carcedo, with Ilias Stothart and Mot Stothart playing Benigno at different ages.
The film runs 105 minutes and mixes drama, horror and thriller elements. On TMDB it holds a rating of 6.2 out of 10 based on 188 votes, which suggests a modest but not dismissive response from viewers. It remains the earliest of the three features Medina has directed.
