Bill Boss runs a state prison in the Arizona desert where violence and defiance are routine, and neither the inmates nor the governor treat him with the respect he believes he deserves. His accountant, Dwight Butler, proposes a drastic solution: stitching the entire inmate population together into a single human centipede, an idea inspired by the two Human Centipede films. Boss takes up the plan as a way to cut costs and restore order, with the prison doctor consulted about the operation.
Tom Six wrote and directed the film, the third in the series he began with The Human Centipede (First Sequence) in 2009 and continued with The Human Centipede 2 (Full Sequence) in 2011. Dieter Laser, who appeared in the first film, plays Boss, alongside Laurence R. Harvey from the second film. Eric Roberts plays the governor, with Bree Olson, Robert La-Sardo and Tommy Lister Jr. in supporting roles.
Released in 2015, the film drew a critics' rating of 3.2 out of 10 from 46 reviews. Audiences were similarly unmoved: on IMDb it holds 2.7 out of 10 from around 23,000 votes, and on TMDB 3.7 out of 10 from 885 votes. Six went on to direct The End of Tom Six in 2026.
